BHRT 101: What Bioidentical Hormone Therapy Actually Is (and Who It's For)
Feeling “off” lately? Are you tired even after a full night's sleep, foggy, irritable, or just not quite like yourself? You're not imagining it, and you're not alone. Hormone shifts are one of the most common and most overlooked causes of these symptoms. They can start in your 30s and 40s, long before most people expect them. That's where BHRT comes in.
What is BHRT?
BHRT stands for bioidentical hormone replacement therapy. Unlike synthetic hormones, bioidentical hormones are structurally identical to the hormones your body naturally produces — think estrogen, progesterone, and testosterone. Because they match your body's own chemistry, they're processed the way your body already knows how to process them.
BHRT is used to help restore hormone levels that have declined or become imbalanced, whether that's due to perimenopause, menopause, thyroid issues, stress, or other factors affecting your endocrine system.
Common signs it might be time to ask about BHRT
Persistent fatigue that doesn't improve with rest
Brain fog or trouble concentrating
Mood swings, increased anxiety, or irritability
Low libido
Weight changes that don't respond to diet or exercise
Sleep disruption
Hot flashes or night sweats
Thinning hair or changes in skin texture
If a few of these sound familiar, it doesn't necessarily mean something is "wrong" with you — it often just means your hormones are asking for support.
How BHRT works at V & Co.
Every BHRT journey starts with bloodwork, not guesswork. We evaluate your hormone panel alongside a full conversation about your symptoms, health history, and goals. From there, we build a plan that's specific to your levels, not a one-size-fits-all protocol.
Hormone therapy isn't "set it and forget it," either. We monitor your levels and how you're feeling over time, adjusting as needed so you're always getting the right support for where your body is right now.
More than symptom relief: long-term protective benefits
Hormones play a protective role in several body systems, and restoring healthy levels can offer benefits well beyond symptom relief:
Bone health. Estrogen plays a key role in maintaining bone density, and the protective effect is well documented. Bone loss accelerates in the years right after menopause, and hormone therapy is one of the most effective tools for preserving bone strength and reducing fracture risk.
Cardiovascular health. Research points to a "timing hypothesis" . Starting hormone therapy closer to the onset of menopause (generally within about 10 years, or before age 60) is associated with cardiovascular benefit, while starting much later may not offer the same protective effect. This is one of the reasons individualized timing and monitoring matter so much.
Cognitive health. Hormones also play a role in brain function, and maintaining healthy levels. Starting at the right time has been associated with cognitive benefits.
The science here is nuanced and depends heavily on factors like timing, dose, delivery method, and your individual health history. This is why BHRT should always be personalized and monitored by a provider, not approached as a one-size-fits-all solution.
Is BHRT right for you?
BHRT isn't only for women in menopause. Many of our patients are in their 30s and 40s and start noticing symptoms well before they expect to. And BHRT isn't just for women, either. We also treat men experiencing low testosterone, with symptoms like fatigue, low libido, mood changes, and difficulty building or maintaining muscle mass.
The best way to know if BHRT is a fit is consultation. We'll walk through your labs, your symptoms, and your goals together, and figure out a path forward that makes sense for you.
